Preventing Diabetic Nephropathy (the Miyako Project)

Project aim

The incidence of diabetic kidney disease (diabetic nephropathy) is very high. Improved diabetic and renal control can help to reduce such incidence. The aim of our project is to introduce to community doctors new concepts, new tools and new medications for diabetic nephropathy management and to empower diabetic patients in the handling of the disease.

The Hong Kong Nephrology Group and her partners are going to start the Diabetic Nephropathy Project 2016 next year.  Its partners included the Hong Kong Baptist Hospital and the Hong Kong Medical Association.

The project composed of three parts

  •  Updating the Diabetic Nephropathy Management Support apps (HKNG)
  • CME in Hong Kong Medical Association Community Districts (HKNG and HKMA)
  • Diabetic Kidney Camp (HKMA, BH, HKNG)

 

The Diabetic Nephropathy Management Support apps

  1. Management of diabetic nephropathy is quite complicated. It involved calculation of the detailed kidney function, usually by the widely used MRDR equation.  The equation can calculate the estimated kidney function (eGFR) from the age, gender , race and serum creatinine of the patient.  The equation is complicated but the management plan depends on it.
  2. We had previously developed an apps which can calculate the eGFR for the doctor and suggested the diabetic and kidney treatment plans. It can be downloaded free of charge.
  3. It is planned that the apps should be upgraded to include the new diabetic drugs, the android version and the Chinese version.

 

The CME in all districts

 It is planned that a CME session should be arranged for each of the community districts of the Hong Kong Medical Association.

 

World Kidney Day

The aim of the World Kidney Day is arouse people’s attention to kidney disease.  In the run up to the World Kidney Day, our chairman, Dr Ho Chung Ping, advised people in Hong Kong to have regular kidney check. This is because the chronic kidney diseases do not have symptoms in the early stage.

The Hong Kong Nephrology Group and her partners will organize a diabetic kidney camp as diabetic kidney disease is the commonest form of chronic kidney disease.

The Diabetic Kidney Camp

Introduction

The Hong Kong Nephrology Group , the Hong Kong Baptist Hospital and the Hong Kong Medical Association are going to organize a  Diabetic Kidney Camp in 20 March, 2016 in the Hong Kong Baptist Hospital.  It will be officiated by Dr Ko Wing Man, Secretary for Food and Health.

The idea of organizing a Diabetic Kidney Camp is that diabetes mellitus is common in Hong Kong and one of the dreaded complication is diabetic nephropathy.  Studies in HA showed that 46% of the patients on dialysis suffered from diabetic nephropathy and the incidence in the private HD patients is much higher.

Good control of diabetes mellitus at the early stage can prevent the diabetic complications and every effort should be made to achieve this target.  Apart from medications, patient education is also important.

Previously we had organized talks in the Community Service Days but the effect is not marked. A much better way is to organize a diabetic kidney camp in which the patients and their relatives can stay for half a days with talks, medical checks and dietary education.  The participants will share the breakfast and lunch in the camp.

We believe this ‘innovative’ project will help to improve the diabetic and nephrology service in Hong Kong.  It would serve as a pilot model for other organizations.  It will be a new initiative and might be included in the National Kidney Foundation KEEP Project.
